The next step in our evolution
The past four years have brought unprecedented challenges to the health care system. With any challenge, comes opportunity. The opportunity to demonstrate transformational leadership, to be bold, try new things, and establish new partnerships that contribute to improving the health of the communities we serve. We are proud of the successes of our Care Beyond Our Walls strategy and how it guided Oak Valley Health through the most difficult days of the COVID-19 pandemic.
Now we’re setting our sights firmly on the future. Our future is the highest quality integrated care.
The health care landscape is changing rapidly, and we look forward to leading the way in reimagining the delivery of care. In the development of our new strategic plan: Integrated Care for Healthier Communities, we listened to and engaged the people who work at Oak Valley Health, the patients and families we serve, and our community partners. We asked ourselves tough questions like:
What became clear was that the health care of tomorrow must have a renewed focus on getting patients the care that they need, at the right time and in the right place. Hospitals and community-based health care services need to work hand in hand to provide a seamless journey for patients and families and improve health outcomes. This is our vision: integrated care for healthier communities.
To achieve our vision, we will stay focused on three key priorities:

- Connections – addressing challenges with creative solutions and partnerships
- Growth – growing our programs, services, academics, and space with innovation in mind

All hospitals strive to provide a good patient experience, but at Oak Valley Health we are committed to delivering an extraordinary one. This means that what matters to patients, families, and caregivers is truly valued. We are committed to consistent, safe, high-quality care delivered with empathy, compassion, respect, and good communication.

Why are you focusing on integration for the next five years?
Bringing the right care together. At the right time.
Integrated care is the systematic coordination of care that brings together different organizations, health care professionals, and service providers at the right time to support the continuity of high-quality, holistic patient care.
This coordination, coupled with the purposeful adoption of technology, is essential for system integration which aims to reduce costs, increase access to health care services, and, more importantly, improve quality of care.
Integrated care makes the patient’s journey through today’s complex health care landscape easier to navigate. It is people-centred in ensuring that an individual’s needs and preferences guide the approach and requires constant engagement, strong partnerships, and a shared commitment to success.
With a strength in partnership and collaboration, Oak Valley Health is fortunate to have developed and sustained strong relationships with community and system partners that make integrated care possible at a time when system integration is a priority in health care across the world. By focusing on integrated care over the next five years, across all of our priorities, we will help our patients get the care they need, whether in the hospital or at home, and this will contribute to their health and the health of the communities we serve.

Oak Valley Health is on a journey to become a High Reliability Organization (HRO) through the implementation of principles and practices that foster a safe, reliable, learning environment of continuous improvement. As part of this journey, we will focus on improving clinical and patient-related outcomes that reflect leading practices and embed the patient voice in all of our improvement work.

Maintaining financial health enables us to provide high-quality care to the patients and communities we serve. We achieve this by advocating at local, regional, and provincial levels for strategic, sustainable operating investment and infrastructure renewal, as well as strengthening our relationships with our hospital Foundations.
Through creative partnership, we can accelerate initiatives and meet the needs of our patients and families faster.
Technology and state-of-the-art equipment
Technology has become an integral part of health care. Technologies, such as those that make virtual care possible, can enable connectivity for patients and providers while at the same time, accelerate integration.
Equipment enables health care delivery. State-of-the-art equipment can accelerate care delivery in today’s modern world.
Data and analytics
Every day, we collect data which enables us to support the health system. The use of predictive analytics enables providers to anticipate changes in health status much faster, sometimes before there are any obvious signs.
Predictive analytics helps us accelerate our responsiveness and improve health outcomes.
